94-year-old, female (M.A.) admitted to Chelsea Rehabilitation and Healthcare Center from CJW Medical Center where she initially presented after a fall and sustained multiple rib fractures. Patient has a history of dementia, HTN, A-fib, and mood disorder. The patient was transferred to Chelsea Rehab for Therapy Services.
Pain Management
Monitor Vitals Daily
Maintain Safety – Multiple falls in the past year
Medication Management – Amlodipine, Amiodarone, Lidocaine Patch
Upon admission, Patient required maximum assistance with all self-care tasks and transfers r/t weakness, and deconditioning. She received speech, occupational and physical therapy 5 times a week. At discharge, the patient was able to ambulate 300 feet with stand-by supervision and the use of a two wheeled walker.
The patient was discharged safely to Our Lady of Hope memory care unit with her daughter and continues to make gains and work with therapy!
